SendUsPacking 'Contributor': Messages page

This page enables contributors to leave a personal message for the list holders when they purchase a gift.
Contributors just enter their name, email address, and their personal message. This gets automatically sent to the list holders when ever a new gift is purchased.
Contributors are asked to confirm they have read the Terms and Conditions.
Once they are happy with the message they just click the 'Save message' button to continue through the process.
